All minor release forms must be printed in color. All minor release forms must be signed and dated by both parents/legal guardians on page 1. Both signatures must be witnessed by public notary, he or she must sign and seal where designated on the form. All signed and notarized minor release forms must be the original document no copies will be accepted.
